WHO DOES YOUR ORGANIZATION SERVE?
Hospice by the Bay’s mission is to provide the highest quality hospice, palliative and grief support services to individuals and families in Marin, San Francisco, N. San Mateo and Sonoma counties and the City of Napa. We are proud to say that no hospice-appropriate patient is ever turned away, regardless of his or her ability to pay.
WHAT IS THEIR BIGGEST CHALLENGE?
At Hospice by the Bay, we witness first-hand the difficulties that families face when they have to guess about their loved ones’ wishes for care at the end of life. Making your wishes known ahead of time is one of the greatest gifts you can give your family. These conversations ensure that a plan is created that reflects the care you want at the end of life. Sharing your thoughts creates a guide for your loved ones if and when they need to make decisions on your behalf. Planning in advance gives you an opportunity to think about what you want for the future. Let us help you start this very important conversation.
HOW DO YOU HELP THEM?
- Getting Your Thoughts Together- On our website, we provide a guide that helps you think about what's important to you as well as how you want to live the end of your life.
- Conversation Starters- Our thought-provoking ads were developed as a good way to start talking. Simply share them with someone you care about and ask them what they think.
- Seminars- Ensure your health care wishes will be carried out, even if you can't advocate for yourself. Learn how to document your choices, choose the right person to make decisions for you, and communicate your values and goals about medical treatment choices.
- Resource Library- As part of our mission of serving the community, we produce many helpful publications and other materials about hospice care and grief support for all ages.
